A Short Guide to Your Appointment at Hollywood GP
New Patients
Welcome to Hollywood GP!
Meeting you for the first time is important to us, which is why we book all New Patients for 30 minute appointments for your first visit. This is so your GP can understand your history, address any concerns and build a strong foundation for your ongoing care.
We ask that all new patients arrive to their appointment 5-10 minutes earlier than scheduled to allow time to complete the registration process. New Patient forms can be found HERE and brought into your first appointment, or filled out on arrival at the practice.
Patients wanting their records transferred can request a transfer of records form via reception.
